PCN DES — which indicators are you finding hardest to hit this year?

We're a 6-practice PCN covering 52,000 patients. The social prescribing targets are well within reach, but we're struggling with the structured medication reviews indicator and the enhanced health in care homes framework. Our care homes are small and our Clinical Pharmacist capacity is stretched. What are others doing to hit the EHCH indicators without additional headcount? And has anyone found a way to batch SMRs efficiently?
