### SAFE

- [ ] Safeguarding lead identified and training up to date for all staff

- [ ] Significant Event Audit log up to date (aim for at least 12 per year)

- [ ] Medicines management policy reviewed within 12 months

- [ ] Controlled drugs register maintained and audited

- [ ] Sharps disposal compliance audit completed

- [ ] Emergency equipment checks documented (defibrillator, oxygen, anaphylaxis kit)

- [ ] All staff DBS checks in date

- [ ] Recruitment checks file: right to work, references, identity verification

### EFFECTIVE

- [ ] Clinical audit programme with at least 2 completed audits showing improvement

- [ ] QOF and Enhanced Services records available

- [ ] Referral pathways documented and reviewed

- [ ] Care planning for patients with complex needs

- [ ] NICE guidance implementation evidence

### CARING

- [ ] Patient satisfaction data (FFT, GP Patient Survey) reviewed and acted on

- [ ] Complaints log with resolution documented

- [ ] Reasonable adjustments log for patients with disabilities

- [ ] Interpreter/translation service access documented

### RESPONSIVE

- [ ] Appointment access data available

- [ ] Triage process documented

- [ ] Did Not Attend (DNA) management policy

- [ ] Out-of-hours/urgent care signposting

### WELL-LED

- [ ] Practice vision and values documented and communicated

- [ ] Staff meeting minutes for at least 6 months

- [ ] Staff survey results and action plan

- [ ] Partners' meeting minutes

- [ ] Business continuity plan

- [ ] Information governance policy and data protection register

### Practical Tips

- Prepare an evidence folder (physical or digital) for each KLOE area

- Brief all staff on their role in each area before the inspection

- Inspectors often speak to reception staff without a manager present — ensure all staff can answer basic questions confidently
