### How to Use

### Exception Reporting Guidance

- Exception rates above 10% in any indicator may be queried by ICB

- Always document the clinical reason for exception reporting

- Common valid exceptions: patient declined, contraindication, informed dissent

### Income Calculation

Each QOF point is worth approximately £213 (2024/25 value — confirm with your ICB).

**Estimated income = Points achieved × £213 × your practice achievement adjustment factor**
