## GP Practice Staff Survey Template

**Recommended frequency:** Annually (ideally same time each year for trend data)

**Target response rate:** Aim for 75%+. Anonymous surveys consistently outperform named surveys.

1. My workload is manageable on a typical day. (1–5 scale)

2. I am able to take my breaks. (1–5)

3. I feel under excessive pressure at work. (1–5, reverse scored)

### Section 2: Wellbeing

4. I feel valued as a member of this team. (1–5)

5. I would recommend this practice as a good place to work. (1–5)

6. I have experienced or witnessed bullying or harassment in the last 12 months. (Yes/No — if yes, please describe)

### Section 3: Communication

7. I receive enough information about changes that affect my work. (1–5)

8. I feel able to raise concerns without fear of consequences. (1–5)

9. My manager listens to and acts on my feedback. (1–5)

### Section 4: Development

10. I have had an appraisal in the last 12 months. (Yes/No)

11. I have access to the training and development I need. (1–5)

12. I can see a career progression route within primary care. (1–5)

### Analysis Guidance

- Calculate mean scores for each domain

- Flag any mean below 3.0 for priority action

- Share results with all staff within 6 weeks
